Below you will find downloadable Documents, Forms & Manuals each Chapter needs to either review or submit. Adobe’s Acrobat Reader is required to view some documents in PDF format.
Collegiate Chapters
- AM Registration Card (have each new associate member fill this form out. Submit the forms along with a New Associate Member Roster and AM fees)
- Career Guide (by e-mail request to the executiveoffices @ phikaps.org)
- Certificate of Initiation with Correct Processing
- Chapter Officer Reporting (electronic, due September 21 / January 15)
- Debit Authorization Form
- Document of No-Hazing (due September 21 / January 15)
- Expulsion Form (by e-mail request to the executiveoffices @ phikaps.org)
- Father Weisenberg Award Nomination
- Individual Affiliation Form (for when a Brother transfers schools and wants to affiliate with the chapter at his new school)
- New Associate Member Roster with Procedures
- Phi Kappa Theta Songbook
- Philanthropy Event Reporting Form
- Risk Management Compliance Form (to be signed and returned every semester)
- Statement of Treasurer’s Duties (due September 21 / January 15)
- Summer Officer Form (due April 30)
- Voluntary Withdrawal and Suspension Forms (by e-mail request to the executiveoffices @ phikaps.org)
